  We offer a national service at the Maudsley and Bethlem Royal Hospitals which specialises in offering treatment to people with Obsessive Compulsive Disorder (OCD) and Body Dysmorphic Disorder (BDD). We are conducting a survey looking at the experiences of people with OCD/BDD who have (a) not benefitted from treatments locally, or who have benefitted but not maintained their progress, and (b) are seeking referral/re-referral toa specialist service but have come up against obstacles.
If you have found obtaining the right treatment for your OCD or BDD difficult, we would like to ask you to complete the following survey. We believe the information will be helpful in informing us as to how obstacles can be overcome.
We are interested in your experiences, whether you are an adult, or a child/adolescent.

IF YOU ARE GETTING BETTER OR HAVE NOT ENCOUNTERED DIFFICULTIES IN BEING REFERRED TO A SPECIALIST CENTRE FOR YOUR OCD/BDD THEN PLEASE DO NOT COMPLETE THIS SURVEY.
Also, please only complete if you live in the UK and are eligible for NHS services.

We have requested you name and contact details so we can contact you for a follow up questionnaire in the future to find out how you are progressing in overcoming any obstcales. We have also requested the name and contact details of your consultant/therapist as it may be useful for us to write to them regarding the obstacles you have met.

By clicking on the tab which moves you on to complete the survey we assume you have given consent for us to use the information provided in the survey for our study. Any reports generated from the study will not use your personal details.

It is also important we emphasize that completing the survey does not guarantee you will be treated at the specialist centre.

If it has been some time since you attempted to be referred to the specialist centres, we would like to know what has been happening in the time since this attempt, and would be grateful if you can provide details of this as far as possible when completing the survey.
